• Gather all the information required on the nomination form before you begin filling it out, since you cannot save your form and return to it at a later date.
• When describing your candidate’s exceptional service, sacrifice or innovation, ask yourself what’s exceptional – above and beyond what they’re being paid to do.
• Give examples that represent their exceptional service, sacrifice or innovation.
• Make sure that at least some of their exceptional acts occurred in 2009.
• How has your candidate’s exceptional act helped others – patients, patients’ families, co-workers, etc? (Are there others you can interview about your candidate – that may provide helpful information? This could include your candidate’s supervisor or co-workers.)
