2010 Official Inspired Comfort Award Rules

Nominate any non-physician professional working in healthcare in the U.S. who impacted the lives of others through exceptional service, sacrifice or innovation during 2009. If the person you nominate is judged as the Grand Prize Winner, you win a prize package of Cherokee Uniforms scrubs, Cherokee Footwear (suggested retail value: $1,000) and a year’s subscription to Scrubs Magazine, the nation’s first lifestyle magazine for nurses! No purchase necessary to enter or win. The Cherokee Inspired Comfort Award is open to legal residents of the United States. Participation constitutes entrant’s full and unconditional agreement to and acceptance of these Official Rules. The Cherokee Inspired Comfort Award nomination begins at 12:01 a.m. Pacific Time (P.T.), March 1, 2010, and ends at 11:59 p.m. P.T., June 30, 2010 (Nomination Period). The contest is sponsored by Cherokee Uniforms, a division of Strategic Partners, Inc., 9800 De Soto Ave., Chatsworth, CA 91311 (Sponsor).  Cherokee Uniforms reserves the right to extend the nomination deadline at its discretion and without advance notice.

Eligibility:

Nomination is open to all non-physicians who are employed in any aspect of patient-care services in the United States who are age 18 and older and who are legal residents of the United States for the duration of the contest. The action or innovation for which someone is nominated must have occurred in the 2009 calendar year. Employees, officers and directors of Sponsor, its parent companies, investors, affiliates, subsidiaries, advertising, promotion, and fulfillment agencies, their immediate family members and persons living in their same household are not eligible to win this award. Self-nominations are ineligible to win. Void where prohibited. All federal, provincial, state, and local laws and regulations apply.

How to Enter:

To nominate someone for this award, the nominator must fully complete the nomination form located under the award section on the website www.InspiredComfort.com. All nominations become the exclusive property of Sponsor and will not be acknowledged or returned. Description of nominee’s accomplishments must be at least 100 words in length and should not exceed 750 words.

Prizes:

All judged winners will receive a Cherokee Inspired Comfort Award trophy, a wardrobe featuring Cherokee Uniforms and Cherokee Footwear with a retail value of approximately $1,000, a 14K gold-plated commemorative pin, and a year’s subscription to Scrubs, the nation’s first lifestyle magazine for nurses.  Grand prize winners will receive an all-expense-paid Caribbean cruise for two. Up to one selected winner in each nomination category will receive the Grand Prize (approximate retail value: $5,000). Grand prize winner’s package includes: One cruise for two persons to the Caribbean or the Mexican Riviera for approximately five days, one room aboard ship, double occupancy, and roundtrip air transportation for two persons in coach cabin from the winner’s nearest major airport to gateway.

Additional prize winners in each nomination category will receive one all-expense-paid trip to a 2011 medical or professional conference in the continental U.S. of the winner’s choosing. Cherokee Uniforms officials must approve of the conference in advance.  This prize includes roundtrip air transportation for the winner in coach cabin from the winner’s nearest major airport to an airport serving the metro area in which the conference takes place, ground transportation, meals, accommodations in a hotel located near the conference, conference admission, and a one-year membership to the professional or clinical association of the winner’s choice. 

Travel Requirements:

All travel reservations must be made at least two months in advance of the departure date and are subject to availability. Travel must be completed by December 31, 2011. No extensions will be granted. Other travel restrictions and blackout dates may apply. Once the trip is confirmed the departure date cannot be changed, or the recipient will forfeit the trip portion of their prize. No alternative prize, cash or other substitution is permitted except at Sponsor’s discretion. Prizes are nontransferable and cannot be exchanged for cash. All federal, state, and/or local taxes are the sole responsibility of the winners. Winners, at winners’ option and expense, may purchase travel upgrades.

Selection of Winners:

A panel of judges will determine winners from all eligible entries received during the Nomination Period. All decisions are final and binding. Odds of winning will be determined by the judging selection and the number of eligible entries received; based upon the anticipated number of entries, odds of winning the Grand Prize are approximately 1 out of 2,000. Winners will be notified by telephone, mail or email within 45 days of selection. Winners will have 10 days from notification to accept the prize by telephone or email. If the prize is not accepted within 10 days from the date of notification, another winner will be chosen. Winners will be announced on the Cherokee Inspired Comfort Award website (www.InspiredComfort.com) on or around September 30, 2010.

General Terms and Conditions:

By entering your nominee, and agreeing to your name being entered into the contest for consideration as a winner, each participant agrees to be bound by these Official Rules and the decisions of Sponsor, which shall be final. Prize winners and those who nominate them agree to give permission for the use of their photographs or likenesses and the details surrounding the service, sacrifice and innovation for which they were judged winners for purposes of advertising and promotion. Sponsor, agents and representatives of Sponsor, its parent companies, investors, affiliates, subsidiaries, advertising, promotion, publications and websites, and fulfillment agencies, and legal advisors are not responsible for and shall not be liable for: (I) late, lost, delayed, damaged, misdirected, incomplete, illegible, or unintelligible entries; (II) any condition caused by events beyond the control of the Sponsor that may cause the contest to be disrupted; (III) any injuries, losses, or damages of any kind arising in connection with or as a result of the prize, or acceptance, possession, or use of the prize, or from participation in the award; or (IV) any printing or typographical errors in any materials associated with the award. Sponsor reserves the right, in its sole discretion, to suspend or cancel award at any time. Guest agrees to release and hold Sponsor and the employees, officers, directors, shareholders, investors, agents, representatives of Sponsor, its parent companies, affiliates, subsidiaries, advertising, promotion, and fulfillment agencies, and legal advisors, harmless from any and all losses, damages, rights, claims and actions of any kind in connection with the contest or resulting from acceptance, possession, or use of any prize, including without limitation, personal injury, death, and property damage, and claims based on publicity rights, defamation, or invasion of privacy.
